IF YOU DON'T HAVE A TEAM, WE WILL FIND A TEAM FOR YOU.

Registration Fee is $140 which includes the jersey. Returning players may use previous official FABA shorts. New players are required to purchase official shorts for an additional $20. We accept payments of CASH, CHECKS and CREDIT CARDS.

You can submit your reservation online by filling out the Player Online Registration Form and click SUBMIT.

Players may also download and print the forms by clicking the link below. You may then submit your completed forms with your payment at the Sunday Games or at Open Gym (See FABA Chicago Officers).
